I give my permission for said child to take part and to participate in a sports program of the Owosso Home School Sports (OHSS), which is a Michigan Non-Profit Corporation.

I understand that serious injury could result to my child from his/her participation in a sports program and I further agree on the behalf of my heirs, executors, administrators, successors and assigns of the undersigned, to release, remise, acquit and forever discharge OHSS, First Church of God (The Hub) where they play and practice, their employees, agents, coaches, volunteers, heirs, executors, administrators, successors and assigns, of and from any and all claim, actions, causes of actions, demands, rights, damages, costs, injuries or other damages whatsoever, which I or my minor children, may have resulting from or occurring during my minor child’s participation in a sports program.

Further, I acknowledge that OHSS recommends that I obtain a full physical examination for my child prior to participation in a sports program, and I further agree on behalf of my heirs, executors, administrators, successors and assigns of the undersigned, to release, remise, acquit and forever discharge First Church of God (The Hub) where they play and practice, their employees, agents, coaches, volunteers, heirs, executors, administrators, successors and assigns, of and from any and all claim, actions, causes of actions, demands, rights, damages, costs, injuries or other damages whatsoever, which I or my minor children, may have resulting from or occurring from my failure to obtain a physical examination.

Christ-Like Character Contract

We have received, read, and understand the Athletic Handbook (download by clicking this link) of Owosso Home School Sports association. We commit ourselves to abide by the policies and procedures as listed in the handbook. We further acknowledge the Board of Directors authority to implement the observance of these policies. We covenant with them to develop Christ-like character.
